Required Paperwork:
- Adult Roster Form ( ALL teams must submit this form )
- Adult Waiver Form ( ALL players must submit this form. All forms must be originals, no fax or scanned copies )

All adult soccer players are required to be "carded" with a soccer ID card produced by the Maryland SoccerPlex & Adventist HealthCare Fieldhouse (formerly know as the Discovery Sports Center). Only carded and rostered players are permitted to play in any match.
To get your card:
- Come to the Adventist HealthCare Fieldhouse front office
- Bring a photo ID and $5.00
- Bring the completed waiver form

In an attempt to make the player card process easier, you must fill out the new 2021 waiver, show some form of ID like your driver's license and bring your old player card, and pay $5.00 to receive your new card. Just as important, everyone needs to come in between 9:00 am and 7:00 pm for those who have not taken care of this matter. Anyone needing a new card please have them call us before coming in to make sure we can take care of their card at the appropriate time and we don’t “shut down” the card printer!!
Finally, please understand that anyone trying to get a card 15 minutes before the game probably will not be playing!!! (So please pass on this information to your teammates!!!!)
